Our experienced clinicians can complete the relevant DVLA medical documentation following an assessment of your medical history and current health. For Group 2 licences, such as lorry and bus driving licences, the DVLA requires a doctor registered with the GMC to complete the D4 medical examination report.

Before your appointment

Please send us your DVLA form in advance wherever possible so that we can check the requirements and ensure we are able to complete the appropriate assessment.

Please bring:

• Your DVLA medical form

• Photo ID

• Details of your current medication

• Relevant medical information or reports, if requested

• Your glasses or contact lenses if you use them for driving

• Your current prescription, where applicable

For some applications, additional information or an eyesight assessment may be required. The requirements vary depending on the type of licence and the reason for the medical.

Firearms and shotgun licence medicals can be more complex to arrange privately because the assessment requires access to relevant medical information from your registered GP practice.

With your consent, your GP practice will need to provide us with the relevant medical records or clinical information required to complete the medical assessment. This may incur a fee from your practice if being sent to us as third party.

We can then undertake the assessment and complete the appropriate documentation where we have sufficient information to do so.

Please note: in some circumstances, it may be more appropriate for the medical assessment to be completed by your registered GP, particularly where access to your full medical history is essential.

If you are considering becoming a foster carer or adoptive parent, a medical assessment is an important part of the assessment process. In England, prospective foster carers are normally required to provide a medical statement, and prospective adopters will be asked to have a full medical examination.
