Annual Health Check
These Terms & Conditions ("T&Cs") govern the use of the Annual Health Subscription ("the Subscription") offered by Shropshire Health Clinic.
1. Eligibility
The Subscription is valid for TWO named individuals and is non-transferable.
Subscribers must be 18 years or older at the time of purchase.
2. Subscription Period & Renewal
This is a monthly payment
The Subscription is valid for 12 months from the date of purchase, unless otherwise specified.
Subscriptions do not auto-renew, unless explicitly agreed at the time of purchase. A reminder will be sent prior to expiry.
No refunds will be issued for early cancellation or unused services, unless within the cooling-off period.
3. Included Services
Subscribers are entitled to the following during the active subscription period:
Gold Standard Membership:
Up to 12 x 15-minute GP appointments
Appointments must be pre-booked and used within the 12-month term. Unused appointments do not roll over. You can use as many appointments as required to create the desired length of consultation.
10% discount on all clinic services and products
Discount applies only during the active subscription period.
Platinum Plus Membership:
Up to 18 x 15-minute GP appointments
Appointments must be pre-booked and used within the 12-month term.
Multiple 15-minute slots can be combined to allow for longer consultations.
Unused appointments do not roll over into the next subscription year.
The appointments can be used by either subscriber.
A comprehensive Annual Health Screen
For both subscribers: Includes a Blood Pressure Check, a Urine Dip, a BMI calculation (with waist circumference measurements if applicable, a blood test that checks a full blood count, liver, kidney and bone profiles, Thyroid, Cholesterol and Hba1c (sugars)). An ECG is also done if indicated.
Further tests or investigations may be recommended with no pressure at an additional cost.

Valid only during the active subscription period.
4. Booking & Cancellations
All services are subject to availability and must be booked via our website, phone, or in person.
A minimum of 24 hours’ notice is required to cancel or reschedule any appointment.
Appointments missed or cancelled with less than 24 hours’ notice may be deducted from your annual allowance.
5. Payment
Full payment is due at the time of purchase unless a monthly payment plan has been arranged.
Accepted payment methods include online payment or in-person at the clinic.
Missed monthly payments may result in temporary suspension or cancellation of the subscription.
6. Termination
We reserve the right to terminate the Subscription (with a proportional refund based on services used) in the event of:
Misuse or abuse of services
Provision of false or misleading information
Clinical determination that care requires external referral or is outside our scope
7. Liability
The Subscription is not an insurance product and should not be used as a substitute for private medical insurance.
Services are subject to clinical judgment, appropriateness, and availability.
We are not liable for delays caused by circumstances beyond our control (e.g. staff illness, emergencies, or regulatory changes).
8. Changes to the Subscription
We reserve the right to update or amend Subscription terms, benefits, or pricing.
Subscribers will be notified in advance of any material changes.
